In case of a medical emergency, and in the event I cannot be reached or present, I give my permission to Creative Keys staff to secure a medical treatment, including hospitalization, for my child.

Full payment is due at the time of registration. Cancellation & refund policy: To receive a full refund, cancellations must be made three weeks before the start date of camp. Cancellations made 20 days or less before the start date of camp cannot be refunded. No refunds will be given due to absences.

Please note: By registering your child(ren) for summer camp at Creative Keys, you hereby give permission for your son/daughter to be photographed or videotaped for promotional purposes. Names will never be used to ensure privacy.
