MusicLink Student Enrollment Form

1. Private lessons are on a monthly membership (subscription-based). Tuition for MusicLink students is subsidized by Creative Keys by 50%. Please note that MusicLink does not provide funding for private lessons, but does check for eligibility and progress. The current rate for tuition is $36 per 30 minute lesson on a monthly subscription - therefore, the full tuition is normally $144 for 4 week months or $180 for 5 week months. At the 50% subsidized rate, the total per student, per instrument will be $72 for 4 week months or $90 for 5 week months. If the student's first lesson will take place after the 1st, the amount requested can be prorated for the remaining lessons in the month. Tuition is automatically processed to your card on file on the 1st of each month for the full month in advance.
Please note that if rates increase in future, the subsidized rate will also increase proportionately.

2. As teachers will be paid regardless of student attendance, tuition is due in full each month, and cannot be reduced for any reason other than the school being closed for a preplanned holiday (lessons missed due to a weather-related closure will be made up). Tuition does not include materials such as books, instruments, or supplies that may be recommended by the instructor. These items must be purchased separately.

3. Lessons missed by the student are forfeited since your lesson time has been reserved just for you. We do offer 45-minute enrichment (group make up) classes once per month that are free of charge. Feel free to contact the office for the upcoming enrichment class schedule and sign up if you have missed a lesson. Enrichment class credits do not expire, so you can attend a future class as your schedule will allow.

4. In the event that your instructor needs to miss a lesson, a Creative Keys guest instructor may teach your lesson at your normal time, or a private make up lesson may be arranged by your teacher.

5. If you decide to take a break from lessons, please inform the front desk in writing (text or email) with as much notice as possible before the first of the month; preferably 10 to 15 days. Your time slot will then be made available to other students for the next month. Please be aware that it may be difficult, or even impossible, to find a time slot available with your previous instructor if / when you wish to return since time slots are in demand.

6. Please arrive on time for your lesson. Since teachers have students scheduled back-to-back, they cannot run over the allotted time.

7. Children must be supervised at all times. If you arrive early, please remain in the lobby with your child until the instructor begins the lesson. Please ensure that you are ready for pick up in the lobby at the end of the lesson time as children may not go out to the parking lot unattended. Siblings of students must remain supervised during lesson times and may not use the kids’ area without a parent or guardian present. Creative Keys assumes no responsibility for children who leave the building unaccompanied and cannot provide child care or supervision before, during, or after classes.

8. Parents, legal guardians of minors, students, and adult students, waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time, including hearing loss or hearing damage. Creative Keys, Inc. is not responsible for any damage to vehicles in the parking lot or items stolen from vehicles.

Throughout the year, we may take photos and video at our location as well as at recitals and other events. Photos and videos may be featured on our website or Facebook page or shared with the media on occasion. Full names are never used to identify students to ensure student privacy, unless permission is given by parent/guardian.
